I �m guessing that Sue �s question is asking about instructions to students, such as, "Make a question to determine whether your partner speaks French." I agree that both "make" and "form" work well. In general conversation, though, I would always use "ask".

It depends if we are referring to an exercise - a written exercise where the children must "make questions" for the given answers.
