Help from a grammar expert, please

Hi,

I m in the process of making bookmarks about the word order of questions to help my students in their oral work.
I ve nearly finished but am a bit unsure as to the correct grammatical terms for what can come in the last part of the section, the bit I would just call "other."

I ve looked on the Net, but can t seem to find a definitive guide.

I wonder if one of you grammar experts would mind taking a look at what I ve done and making suggestions for improvement in this area before  I give them to my students and upload them here.

I would be most grateful for your help.

Anyone who is willing and able to help me, please send your e-mail address to my PM and I ll send the document to you, though it won t be till tomorrow now as I m off to bed.

Thanks in advance for your time and support. It s so nice to have a community of experts like this to ask these questions to!

Good night all.
Julia
