Could you help me out with my survey, please

One of my workplaces in considering getting into ESl for kids and I �ve been asked to do some researches and write a paper about it. I have bought, I think about 8 books from amazon, looked throughout the internet, I �m teaching kids myself, but I �m still missing the experience and much valued advices of other teachers. So I was hoping some of you would help me out, answering some questions. I created a survey on freeonlinesurveys. i have never done that before, so if it doesn �t look professional at all or not well organized, you know why :). But i would really appreciate your help, if you don �t want to or can �t answer one of the questions just put NA.
