This worksheet is a revision for forming questions in the present tense simple and progressive tenses. My students seem to have problems with word order, do, does and so I created this worksheet which hopefully helps them to remember what we have learned so far.

When did you last...?
Students make questions with the expression �when did you last...?� and the verbs and phrases provided. Encourage students to give a complete answer, and if not make more questions related to the facts.
