Hi there. "happened" is OK in this context because it refers to a past event. The fire was obviously a specific thing, like an accident or a football match, that occurred and that is why we can use "happened"...

Most definitely "on." "Happened" is fine and "the" rather than "a" because it happened on a particular Thursday such as an aforementioned week. |
