It �s a play on words - kind of like a pun,
A candle has a flame and the cotton piece that you light is a wick.
An old lover is a an old flame because they made your heart burn with love!
The caption says:
Charlie is stunned to learn his ex-girlfriend is now a man (Chris O �Donnell).
His old flame (his old girlfriend has had a sex change operation and has become a man)
and now has a penis (wick)
