This is a mixer activity. Students look for a person in the class who answers "yes" and then writes that student´s name in the space. This is a great activity for helping students to formulate questions in different tenses. They must ask the questions that will produce an affirmative answer. For example, student A might ask, "Have you ever been to Las Vegas?" If student B answers, "Yes, I have,", student A has to ask the student´s name and write it in the space. I make my students also ask to spell each others´ names.
