The pupils have first to label the picture of the monster with the body parts and write the names of the shapes. Then , they complete the grid in order to create their own monster. When they have finished, they draw their monster and write the sentences using the infomation in the grid (he has got...). My pupils always love this activity!! I found the picture on a book called Activity Box, by Cambridge University Press, if I remember well (but I´m no sure. If you know where the picture is from, let me know).
