The worksheet was designed to be used as a review of Verb to be and the WH question forms associated with it. It does include the Present Simple yet. There are, however, TWO honorable exceptions: "How do you spell your name?" and the alternative to "What are you"/"What do you do?" which is much more frequent and therefore more natural than the first option. I hope this worksheet can be useful to the site.
