I think itīs better to start with the role play and give the visuals as a follow-up work activity at home (or in class as a test) to avoid repetition.
For the preparation of the role play, insist on the fact that they need to build themselves a new identity (name, job) to get them more into their roles.
For the role play, start with the first set of problems (smoking for instance) but avoid having one group talking about their problems and then the other group giving the solution afterwards. Try to have reactions after each problem instead.
With a small group of students you can organise your class so as to form a circle.
Do not forget the language assistant who can be very useful as well.
