Identity forms
I use this document with another worksheet you can download that I entitled "Identity : exercises".

This worksheet here consists in 2 identity forms and in a recap of the prompts the kids need to introduce someone precisely.
The task is for them to introduce the teenagers. The teacher can offer them to pretend they are Kim or Bradley so that they use the first person of the singular. Then, they are told to use the third person of the singular so that they practice the present simple.
If you want to work on both the answers and the questions, you can organize a pairwork activity. Slip the document into two parts. Each kid has one identity form + the prompts. Tell them the two kids are to go to Summer camp. They call the camp supervisor to fill in a registration form. One is the supervisor, and the other student is the teenager. When they are finished, they switch roles.
