It could be used as: 1) test 2) revision worksheet 3) quided speaking -by using the 1st page- You could create a different ID card by deleting some of the information. Students could ask each other questions such as: "What does he do on Mondays?. How old is he? How many..." 4) game- by having the kids look at Nick´s ID card for a while as a memory game 5) writing: after answering the questions or talking about Dad (=Nick), students could write a couple of paragraphs about him.
