Silly or Dangerous activity video survey form for ESL students
Use this form in conjunction with 7 silly/dangerous short videos (1-2 mins) that you have found somewhere. If you have internet access in the classroom, youtube is a good choice. eg AFV videos
The students fill in the survey form after they watch the video. After that, youy can do anything your heart desires. eg write an essay on your dangerous/silly activity, give a speech, mime it out, some sort of group activity etc.
Instructions:
Explain that we will watch some funny/wierd/?? videos. We will also do a survey. After the survey, you will form groups then think of something silly or dangerous you did. Your group will choose the best story, and act it out.
Write scale of agreement (page 3) on the board.
Do a survey with the class. Teacher asks a question. Eg Do you have a mobile phone?
The teacher then points to a response on the scale written on the board. He says – If you think XXX then raise your hand. Etc.
Preteach vocab for the top row of the form. Eg cool, silly, dangerous, stupid.
Explain form to students (teacher to simplify the following to fit class english level).
In each row for the corresponding video, you circle the correct response for each question which is along the top of the form. You can also write in another response if you want. For the next video, move down to the next line.
