Practice Form
I made this sample form because so many of my students have trouble filling out forms correctly. Things that seem to mess them up a lot are the date, their gender and the common abbreviation, DOB (Date of Birth). Also, some students write their addresses incorrectly. We fill the form out together and I explain the vocabulary as we go along.
I also have the students look up words in a bilingual dictionary that they would use to describe their country of origin, so students get a chance to practice those dictionary skills.
