When was the last time you ... ?
A conversation activity. Cut the sheet in half. Put students in pairs: Student A and Student B. Hand out the questions.
Before starting I usually point out two typical structures used to ask and answer the questions:
QUESTIONS
When was the last time (that) you rode a bicycle? (Longer but perhaps easier to use.)
When did you last ride a bicycle? (Shorter but perhaps harder to use. "Last" here is an adverb and precedes the main verb.)
ANSWERS
The last time I rode a bicycle was August. (Longer but easier, again, perhaps?)
I last rode a bicycle in August. (Shorter but perhaps harder.)
Depending on the level of the class, you might mention a third possibility, if the last time was a long time ago:
I havenīt ridden a bicycle for ten years/since I was at school ...
Why not use the questions for individual lessons too, with the teacher taking the role of Student B?
