Extensive Reading Cover Page
To keep track of what students are reading. Here is a form you can ask students to hand in after reading each book. This saves you keeping the work the student has done on the book and the grade. You can then file it away until need be. It is also a record to keep track of what students have read in previous years - you just have to hand the file containing these forms to the new teacher each year.
