This worksheet has three different sheets of questions that students can use to conduct a survey - i.e. interview one to three classmates. It is both a speaking, listening and pronunciation exercise as interviewers need to speak clearly and interviewees need to listen carefully in order to ask and answer questions correctly. Make sure interviewers never show the questions to their partners and pre-teach vocabulary or adverbs of frequency if necessary (word bank with adverbs of frequency provided). There is a nearly identical survey with questions for young learners.
