This is a form I use to evaluate an oral presentation. I generally complete a form, giving suggestions or making comments below each category; allow the speaker to use the form for self-evaluation; and allow a classmate to use the form for peer evaluation and suggestions. I chose advanced level but I think it could be used for intermediate level as well.
