I use this self-evaluation with my beginner adult ESL students (Level 0-3, literacy - high beginning). The form can be modified to fit your needs. It provides an opportunity for students to assess their skills and reflect on what they need to work on. My students appreciate a summary of what is needed to "pass the test"!
